Solution recognized for helping health plans measure the cost of individual social risk and estimate and validate return on targeted SDoH interventions

MedeAnalytics, the healthcare performance improvement company, has been recognized by the 2026 Healthcare Innovation Impact Awards with a bronze award for its Social Risk Insights solution, which projects the financial impact of social determinants of health (SDoH) interventions, tracks outcomes over time, and compares actual versus estimated return on investment.

The Healthcare Innovation Impact Awards recognize products, services, and technologies advancing healthcare through innovation and real-world value. Honorees are selected by an anonymous panel of healthcare professionals. Entries are evaluated based on innovation, demonstrated results and the ability to address important challenges across the healthcare ecosystem.

Social Risk Insights helps health plans connect social risk to the outcomes they are accountable for, including quality performance, total cost of care and return on investment. Leveraging Mede Foundation and its AI-native data management platform, Health Fabric™, the solution unifies individual social risk data with medical risk and claims history, pharmacy and financial data so plans can identify stacked risk and better target and prioritize interventions to groups of members (cohorts) with the same overlapping needs, such as diabetes plus transportation barriers plus food insecurity.

For one commercial health plan, social risk stratification identified five high-risk cohorts that claims data alone did not detect. Analysis found that interventions targeting specific social cost drivers across those cohorts could generate up to $25 million in savings with a projected 3:1 return on investment.

The solution combines member-level social risk data from Socially Determined with independent, actuarially accepted methods from Mathematica. These collaborations allow health plans to model projected ROI and intervention costs before committing resources, monitor cost and utilization performance over time to determine whether programs are on track, and compare cohort performance before and after implementation to validate realized ROI.

“Health plans have known for years that social factors shape costs and outcomes. What they have lacked is a reliable way to attach a dollar figure and show which interventions work,” said David Wolf, senior vice president of product management at MedeAnalytics. “Social Risk Insights gives plans that evidence with independent actuarial validation. This recognition shows what is possible when social risk is something a plan can measure, manage and report on with confidence.”

Social Risk Insights also simplifies how data moves between partners. MedeAnalytics and Socially Determined exchange data through secure, direct AWS data streams rather than traditional file transfers, and Mathematica accesses what it needs for actuarial validation through MedeAnalytics’ Health Fabric™ without duplicating datasets. The result is more timely data and stronger security.

The recognition is the third industry innovation-focused award for MedeAnalytics this year, following its selection as Best Overall Healthcare Data Analytics Platform in the 2026 MedTech Breakthrough Awards and listing among Top HealthTech Companies of 2026 advancing healthcare through technology using AI and data analytics.
