The inaugural Index gives procurement leaders segment-level benchmarks for supply spend, formulary performance, and regional trends as teams finalize FY27 procurement strategies

Dentira, the vertical procurement platform built for dental, today released the Dentira Dental Spend Index (DSI), the first dental spend benchmark built on live procurement data from 267 DSOs across the United States. The Index gives Chief Procurement Officers, Chief Financial Officers, and the investors and operating partners who back DSOs a clear view of how their spend compares with segment peers. The full report is available to download today from the Dentira Dental Spend Index page.

The semiannual Index tracks supply spend, formulary compliance, and supplier strategy for DSOs, built from procurement data on the Dentira platform. It arrives as teams set FY27 budgets under sustained cost pressure: since 2021, dental reimbursement has risen 19% while overall inflation is up 27%, according to the ADA Health Policy Institute.

The Index draws on live purchasing, formulary, budget, and invoice activity from 267 DSOs spanning 15,000 locations in the United States. Findings are reported at segment and category level, so pricing stays between buyers and suppliers.

Key findings from the Fall 2026 Dentira Dental Spend Index include:

  • Scale cuts supply spend nearly in half. Dental groups of 101 or more locations spend 4.1% of collections on supplies, against 7.4% for groups of 10 or fewer.
  • Supply prices have climbed since 2021. Every top-10 supply category rose between 2021 and 2025, led by labels and signs at +20.1% and disposables at +15.8%.
  • The Formulary Gap is the largest lever. About half of ordered products sit off formulary, and even Enterprise DSOs order 45% off formulary.
  • Growth is broadening, and the South Atlantic leads consolidation. SMB and Mid-Market DSOs now outpace Strategic DSOs in growth (32% and 26% vs. 16%), and Enterprise DSOs accelerated from 18% to 26%. Among DSOs on the Dentira platform, the South Atlantic represents nearly one in three acquisitions over three years, with Florida, Texas, and California leading all states.
  • Surgical and implant care shapes the supply mix. Implants and bone regeneration materials rank among the top four spend categories in every segment, a clear signal of where specialty services concentrate across DSO portfolios.
  • Control delivers. Dentira customers report reducing supply spend by 1.1 points of collections in their first year on the platform, the equivalent of moving from 8% to 6.9%.

“Every order, invoice, and formulary decision on our platform adds to the most complete picture of dental spend the industry has had,” said Vik Gupta, co-founder and CEO of Dentira. “We built the Dentira Dental Spend Index to help procurement leaders benchmark against their peers and plan with evidence.”

“As a dentist and a DSO founder, I know every supply dollar competes with a new hire, a new operatory, or a new service line,” said Dr. Michael Benarroch, co-founder and Chief Strategy Officer of Dentira. “The leaders I talk with want control of spend and confident clinicians. This Index shows them where to start.”

The report also includes the Dentira Dental Spend Maturity Model and a 90-day plan for procurement teams. For investors and operators who back DSOs, the Index maps where consolidation is happening and quantifies the 3.3-point supply spend gap between small and scaled platforms, a margin lever that integration can capture.

The Spring 2027 edition is expected to add a category-level formulary view, building on questions procurement leaders raised at Dentira’s October preview. Leaders can download the Dentira Dental Spend Index today.
